UPA Psychologist Locator Service is compiled and published by the Utah Psychological Association (UPA) as a reference source of demographic information on individual member-psychologists in the Utah.
The only permitted uses of UPA Psychologist Locator Service are for

1. the personal convenience of individual members of the general public to facilitate quick access to information about psychologists with whom they may be consulting regarding psychological services and
2. individual psychologists or their staff to view their personal listings or those of colleagues for professional purposes such as patient referrals.

UPA reserves the right and hereby notifies you that it will be monitoring all use of this website for compliance with the terms and conditions of this agreement - including the permitted use described above. UPA shall block your use of this product indefinitely should we determine, at our sole discretion that your use is beyond that permitted above. Neither UPA Psychologist Locator Service nor any of its data, listings or other constituent elements may be downloaded, republished, resold, duplicated or "scraped", in whole or in part, for any purpose other than the personal uses permitted above.

The use of UPA Psychologist Locator Service by hospitals, managed care organizations, PPOs or other commercial entities or individuals to verify the credentials of psychologists is prohibited. The database of psychologist information which drives UPA Psychologist Locator Service does not contain sufficient information with which to verify psychologist credentials under the standards of the Joint Commission (JCAHO), National Committee for Quality Assurance (NCQA) or the Utilization Review Accreditation Committee (URAC). Therefore, it is your duty to verify the credentials of any individual or groups of individuals you choose to contact under the guidelines set forth above.
